Insurance Committee NewsOPEN ENROLLMENT IS NOVEMBER 1-30, 2008
During open enrollment you may make plan changes to your dental/vision insurance.
Applications for an elective change in your present coverage will be accepted only in the month of November. Deductions will begin on the December 15th payroll, and the coverage you select will be effective on January 1, 2009.
You may also apply for additional life insurance. The underwriter must first approve this addition life insurance before it will become effective. Deductions will begin upon approval from the underwriter.
Change may be made at any time during the year for a family status change. The application has to be made with 30 days of the event. The family status change can be made for birth of a child, placement for adoption, marriage or any changes to employee or spouse’s employment. Any preexisting condition exclusionary period will not apply to a newborn child or adopted child.
Any employee who wishes to have his/her taxable wages deduced by the amount of health insurance premium may join the Choice/125 cafeteria plan effective with the January payroll. This also applies to those who with to discontinue their enrollment in the Cafeteria Plan.
